Booking Terms & Conditions & Privacy Policy

1. Bookings & Appointments

Appointments can be made via our online system, phone, or email.

A confirmation will be sent once your booking is confirmed. Please ensure your contact details are accurate and up to date.
2. Cancellation Policy (NDIS-Compliant)
In accordance with the NDIS Pricing Arrangements and Price Limits:

We require at least 2 clear business days' notice for cancellations or rescheduling.

Short Notice Cancellations (within 2 business days) or non-attendance may incur a charge of 100% of the scheduled service fee.

Exceptions to this policy include:

Medical emergencies or hospitalisation
-
Family emergencies or bereavement
-
Natural disasters or extreme weather events

Sudden transport disruptions
-
Other exceptional, unforeseen circumstances

Please note: For services funded under the Disability Support Worker (DSW) Cost Model, a 7-day cancellation notice applies.
3. Provider Travel & Charges

We may charge for provider travel in line with current NDIS guidelines.

This includes:
-
Time-based travel charges (from the relevant NDIS support category)
-
Per-kilometre charges (e.g., $0.99/km for vehicle costs)

Travel fees will be discussed with you and agreed upon in your Service Agreement.
4. Invoicing & Payment

Invoices are issued in accordance with the NDIS Pricing Arrangements and your service agreement.

Payment is due within 7 business days, unless other arrangements have been agreed upon.
5. Safety, Consent & Participation

By booking a session, you consent to the service being delivered and agree to participate voluntarily.

Please disclose any relevant health conditions to ensure safety and suitability of the exercise program.

All services are delivered by accredited professionals under the Exercise & Sports Science Australia (ESSA) guidelines.
6. Digital Tools & AI Disclosure (Splose Software Use)
Revive Therapy Services Australia uses Splose, a secure cloud-based platform for practice management, which may include AI-assisted tools.
How We Use AI in Splose:

Formatting clinical documentation

Generating appointment reminders
-
Using custom fields for tailoring client goals
-
Automation to support admin tasks (never clinical decisions)
All outputs are reviewed by a qualified professional before being saved or used. AI does not replace human clinical judgment.
7. Privacy Policy
Revive Therapy Services Australia values your privacy. We comply with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs). As an ESSA-aligned provider, we maintain the highest standards of ethical and secure information handling.
We Collect:

Name, contact info, and DOB

Medical/therapy history

NDIS and funding information

Service notes, reports, and outcomes

Emergency contacts

Other information necessary to provide and coordinate care
We Use This Information To:

Deliver and tailor exercise physiology services

Communicate with you and your team

Meet NDIS and legal obligations

Support your clinical goals and safety
How We Store It:

Digitally via Splose, a secure, encrypted platform

Access is restricted to authorised staff only

Records are retained as required by law and then safely destroyed
Your Rights:

You may request access to your information or ask for corrections

We will respond promptly and transparently

You may also withdraw consent at any time (with service limitations discussed)
