1. About Us
Revive Therapy Services Australia ("Revive", "we", "us", "our") provides mobile Exercise Physiology services across regional and remote Western Australia, including the Pilbara.
By accessing this website or engaging our services, you agree to these Terms and Conditions.
2. Scope of Services
Revive provides Exercise Physiology services including assessment, treatment, education, reporting, and program delivery.
All services:

Are evidence-based and delivered by a qualified Accredited Exercise Physiologist
-
Are general in nature unless part of a formal clinical assessment
-
Do not replace medical advice, diagnosis, or treatment
Revive reserves the right to modify, refuse, or discontinue services where clinically appropriate, unsafe, or outside scope.
3. Bookings and Service Engagement
By booking a service, you agree to:

Provide accurate and complete personal and health information
-
Be available at the agreed time and location
-
Ensure a safe and appropriate environment for service delivery
Revive reserves the right to decline or cease services where:
-
Safety risks are identified
-
Information provided is misleading or incomplete
-
Engagement is inconsistent or non-compliant
4. Fees, Pricing and Adjustments
Fees are charged in accordance with:

Revive Therapy Services Australia private rates
-
NDIS Pricing Arrangements where applicable
Revive reserves the right to:
-
Adjust pricing at any time
-
Apply remote loading, travel, or complexity-based fees
-
Charge for non-face-to-face services (e.g. reports, programming, communication)
All fees are considered accepted upon booking.
5. Payment Terms and Debt Recovery
Payment terms are as follows:

Self-managed clients: payment due within 7 days of invoice
-
Plan-managed clients: invoices issued to Plan Manager
-
NDIA-managed clients: services claimed via NDIS portal
Where invoices remain unpaid:
-
Services may be suspended immediately
-
Outstanding balances may incur administrative fees
-
Accounts may be referred to a debt collection agency
-
You may be liable for recovery costs, including legal fees
6. Cancellation, Non-Attendance and Late Notice
Revive operates a strict cancellation policy due to high demand and remote service delivery.
Minimum notice required: 48 hours
If insufficient notice is provided:

Less than 48 hours: up to 100 percent of the scheduled fee will be charged

Non-attendance: full session fee applies
-
Late arrival: session will end at the scheduled time and full fee applies
Cancellations must be made via phone, email, or confirmed written communication.
Cancellation fees apply in accordance with NDIS guidelines where relevant.
7. Travel and Remote Service Conditions
Revive operates across regional and remote areas.
You acknowledge that:

Travel time and costs may be charged
-
Scheduling may be impacted by weather, access, or logistics
-
Grouping of clients may occur to optimise service delivery
Where access is not possible or unsafe, services may be rescheduled or delivered via telehealth.
8. Workplace and Corporate Services
For corporate, workplace, or on-site delivery (including mining and FIFO environments):
The engaging organisation agrees to:

Provide a safe working environment
-
Ensure participants are medically cleared where required
-
Accept responsibility for implementation of general advice
Revive provides education and general guidance only unless individual assessments are conducted.
Revive is not liable for workplace injuries or outcomes resulting from implementation of general advice.
9. Health, Risk and Participant Responsibility
You acknowledge that:

Exercise involves inherent risks, including injury
-
You voluntarily participate in all activities
You agree to:
-
Follow professional advice and prescribed programs
-
Disclose all relevant medical conditions
-
Cease activity if unsafe or advised
Revive is not liable for injury where:
-
Advice is not followed
-
Health information is withheld or inaccurate
-
Activities are performed outside professional guidance
10. NDIS Participants
For participants accessing services under the National Disability Insurance Scheme:

Supports must meet reasonable and necessary criteria
-
Participants are responsible for ensuring sufficient funding
-
A Service Agreement may be required prior to service delivery
Revive reserves the right to cease services where funding is unavailable or approvals are not in place.
11. Privacy and Data Protection
Revive handles personal and health information in accordance with the Privacy Act 1988.
Information will be:

Stored securely

Used only for service delivery and communication
-
Shared only with consent or where required by law
12. Limitation of Liability
To the maximum extent permitted by law:

Revive excludes liability for indirect, incidental, or consequential loss

Liability is limited to the value of services provided
Revive is not responsible for:

Outcomes outside its control
-
Third-party actions
-
Misuse of advice or programs
13. Intellectual Property
All materials, programs, resources, and website content remain the property of Revive Therapy Services Australia.
You may not reproduce, distribute, or use materials without written permission.
14. Termination of Services
Revive may suspend or terminate services immediately where:

Payment obligations are not met
-
Safety concerns arise
-
Boundaries are breached
-
Engagement is inconsistent
Where possible, 7 days’ notice will be provided.
15. Changes to Terms
Revive reserves the right to update these Terms and Conditions at any time.
Continued use of services constitutes acceptance of the updated Terms.
16. Governing Law
These Terms and Conditions are governed by the laws of Western Australia.
